To Patients New to Our Practice:
We appreciate you for selecting us to advise you of the placement of periodontal implants and your periodontal health. We are grateful for the opportunity to serve you and will call you 1-2 days prior to your scheduled appointment. If we are unable to reach you and you cannot keep the appointment, please contact us at least 48 hours before your appointment to reschedule. When we receive cancellations, we will make every effort, if requested, to give you an alternative appointment. We would greatly appreciate your completing both sides of your form before visiting and bringing it with you. This will assist us in giving you appropriate attention during your time here.
Before meeting us for Implant Placement/Periodontal Therapy, you may spend a few quiet moments reviewing your medical and dental history as well as how it might have affected your present periodontal status. You might also consider what you desire for your dental health, what you expect from us as your Periodontist, and what questions you need to be answered.
As for financial arrangements, if you have dental insurance, we will be happy to help you with insurance submission and will accept the assignment of benefits for certain plans. Prior to treatment, we will submit a diagnosis and proposed treatment plan (pre-treatment estimate). We will work with you to ensure the maximum benefit to which you are entitled. We realize, however, that dental insurance is an advantage that not all patients possess. We try our best to work with patients in all financial circumstances. Payment of fees is confirmed with our finance and treatment coordinator prior to commencing a course of treatment. Alternative finance options are available upon request.
